Radon Daycare Project- Survey Contest

Official Contest Rules & Regulations

 

Who Can Enter

  • To be eligible to participate in this contest, entrants must be either a daycare/childcare owner/operator or staff in Nova Scotia, or a parent whose child currently or previously attended daycare or childcare in Nova Scotia.

  • All eligible participants must currently reside in Nova Scotia.

 

How to Enter

  • Complete the designated survey via Survey Monkey and provide name and contact information

  • To enter, complete the designate survey for either daycare/childcare owner/operator or staff in Nova Scotia or for parents whose child currently or previously attended daycare or childcare in Nova Scotia. The designated survey MUST be completed its entirety and provide their name and contact information

  • The contest starts on September 13, 2021 and ends October 31, 2021 (4pm) and all entries must be received by The Lung Association of Nova Scotia by October 31, 2021 (4pm) ADT. The Lung Association of Nova Scotia is not responsible for lost, late or misdirected entries, server availability, computer or any other electronic malfunction.

  • On November 1, 2021 two (2) winners will be selected for the grand prizes and twenty-five (25) will be selected for the remaining prizes. Winners will be selected by random drawing from among valid entries.

 

The Prize(s)

  • Two (2) winners will be selected for the grand prize $500 Visa Gift Cards. Winners must have completed the survey via Survey Monkey.

  • Twenty-five (25) winners will be selected for the prize of $50 Grocery Gift Card. Winners must have completed the survey via Survey Monkey and provided their name and contact information

  • Each winner will be notified by phone and/or email. Before claiming the prize, the winner must provide his/her correct name, address and telephone number to The Lung Association of Nova Scotia at the time of notification. Each winner must sign and return a release and release form, allowing us to publish information about the winner. The Lung Association of Nova Scotia reserves the right to examine additional identification and may choose to accept or deny awarding the prize based on the identification presented.

  • If the winner cannot be notified within fourteen (14) days, or does not timely sign and return the release form, then the prize will be forfeited, and the prize will be awarded to an alternate winner randomly selected from among the remaining valid entries.

  •  No transfer or assignment of prizes is allowed.

  • Only one prize will be awarded to each winner (name can only be drawn for one prize).
